When you click the button, you will either gain or lose items. Fruit and water (clean added to dirty, clean priority, dirty secondary but decreases luck) will be subtracted from per minute. Any idem that's negative will decrease luck. Better luck (above 0) will decrease amounts of bad things, but worse luck will decrease amounts of good things.
