[ form for people without DID ] username / only one preferred name / pronouns, gender, & sexuality / three or more main traits of your personality -- [ form for people /with/ DID ] username / preferred name your most common identities or who you're most comfortable with, or who most commonly fronts, or your alters / those identities' genders, pronouns, and sexualities / three or more main traits of those personalities -- and so you shall be added!! be sure to add any other notes about you or your alters if you need to!!
