Click dat green flag twice! At the first stage, click the cupcake case to change it's colour, and all the flavour icons around it change it's flavour, so click them to change that. You can then drag a bunch of toppings and sauces onto your cupcake! If the music spazes out for you, press space to stop the noise! :D
