Put in your worm costumes in this order: head, body, tail. See the costumes of the worm sprite for details. If you want to use a worm, use my project 'Worm Engine 7' (link below) with the value from here in the last non-variable value in the 'move () steps' block in the body/tail physics script. To generate, go into the 'generator' sprite and run the 'generate with worm type ()' block with the type being your worm type. Worm types point towards the three worm costumes.
