This is Rainstar. She's a rogue who entered Windclan as an apprentice. Her warrior name was Rainstorm and her idol was Fernstar, but when she became a deputy, the Clans broke into a war. This was the end of days and the clans went their separate ways. She had fallen in love with Witherheart from Shadowclan, so that's why she's saying, "Don't Forget Me". Starclan came to her to tell her that she needed to bring the clans back together before it was too late. She had become pregnant and had Witherheart's kits, then she went to go save the Clans. She brought the clans back together and was reunited with Witherheart. Witherheart joined Windclan and they lived together with their kits, Neverkit, Fernkit, and Marchkit.
