Drag the outfit or accessory onto the girl to dress her. If you want to add some thing else on that goes on top of it ,for example if I wanted to put on the trousers but I'd already put the shorts on, then drag the other item ,the shorts, off the girl and replace it with the other thing. Also, click the item to change its costume (Warning, the shirt has only one costume). Also, click the backdrop to change its look and the "Backdrop Colour" button to change the backdrop colour!
