Just click what you want to put on. I recommend you Put pants before shirts but if your doing skirts then do skirts before the shirts lol. Do the hair and then the eyes. You can do the mouth whenever. Shoe after pants. Hats after hair. Then push done. If you have used Gacha Club you will realize that yes this is similar to the real game. You change the background once you push done. Also when you push done you will see that is says icon. In the real game gacha club lets you pick icons for each character on the profile. So i did that on scratch to! Also Their are boys stuff to not just girl things.
