It's very simple! You just use either w, a, s, d or ↑, ←, ↓, → to steer the car(it will always move forwards) point the mouse to aim and click to shoot, you get points for hitting the people(some people will have more health and just shift colors when hit, but you will still get the point). You lose health by amount of people who have seen you shoot multiplied and gotten past you, since you don't lose health from other people shooting you, the "health" just shows how close police is to catch you, when you hit 0 you will go to the shop where you can buy upgrades for the points you got in the round. you can also press space to go to the shop. BTW bombs will begin spawning when you hit "level/speed" 10, if you want to play with bombs just set the difficulty to 10 or higher
