Up arrow to move forward, left and right arrows to turn. A and D to strafe. Higher FOV and lower resolution will cause more lag Space to show the map and player W to sprint.
Did this in about an hour, did most of it myself but looked at Griffpatch's tutorial when I was completely stuck. This means it is basically a modified version of Griffpatch's code. Most lag inducing settings: Resolution 1, FOV 60, any ground shading apart from 1, GS:Accuracy 1, sky shading walls, SS:Accuracy 1.
